Claim Submission Documentation Required -

1. the signed copy of the purchase agreement.
2. The itemized paid invoice(s) from adaptive equipment company (must state customer’s name, eligible Hyundai vehicle, and Hyundai Vehicle Identification number [VIN[).
3. the copy of the vehicle registration
